An excellent study from Sydney has demonstrated that people who are in need of home medicines reviews are not necessarily achieving access to this valuable service. Although HMRs were used more frequently amongst those with polypharmacy and other markers of high-risk prescribing, over 90% of the older people with these markers did not receive an HMR over a 5-year period: for example, an HMR was received by only 8.3% of participants dispensed ≥5 medications when measured over a 5 year period.
