WHO Medication Without Harm

The World Health Organization (WHO) have launched Medication Without Harm, as a part of their Global Patient Safety Challenge. One excellent resource that can be accessed (HERE) is a Powerpoint presentation that addresses Medication Safety – this identifies high risk drugs,  provider and patient factors that impact upon medication safety, and systems strategies to reduce risk. One excellent aspect of the presentation is a slide that summarizes key strategies for medication safety. The presentation also covers aspects like polypharmacy, issues at transitions of care, formal structured medication reconciliation processes and more. The presentation is certainly recommended reading for anyone with an interest in medication safety.
